Class Drop Request Form
A completed Class Drop Request Form is required to withdraw from a class.
Drop requests become effective immediately upon receipt. If you wish to complete the current month, please wait until after your final class to submit your form.
No refunds or credits will be issued for classes remaining in the month once a drop request has been submitted.

Students remain enrolled until a completed Drop Request Form is received and processed. Verbal or telephone drop requests cannot be accepted.
Registration Form
All students enrolled in a class must have a registration form signed by a parent. All those participating in open gyms, birthday parties, field trips and other special events must have a registration form signed by a parent.
